EVALUATION (Work Assessment)


DEFINITION: this is the initial rehabilitation program into which most clients enter.  It is designed for the adults (age 18 or older) with a severe disability.  Its basic design and function were to meet the needs of clients sponsored by the Arkansas Rehabilitation Services and the Division of Blind Services.  It can also be initiated for other eligible clients.  It is a short-term(10-day) in-facility involvement in work areas, counseling and testing areas, and work socialization areas that culminate in definitive statements relating to the individual's capacity for work and potential to benefit from further services.  It is an investigative process directed toward identifying and measuring the client's work-related behavior in order to determine the need for placement or additional rehabilitation programming.


PROGRAM GOAL


The goal is to have a program and staff that will have an accurate and efficient method to identify and measure the client's work-related behaviors which will form a solid foundation for recommending additional programming. 


PROGRAM OBJECTIVES


There are several objectives of Evaluation and they vary according to the individual client.  We attempt to answer any and all specific referral questions posed by the referral source, to complete an Evaluation Form to help formulate specific statements regarding the client's work-related behaviors, to determine the client's ability to adjust to the workshop schedule, to determine average productivity, to identify major problems or problem areas detrimental to employability, to develop a strengths/needs list, to determine need for placement, to formulate initial program planning with the client for those who need additional programming, and to help place those clients who are ready for placement.

WORK ADJUSTMENT TRAINING

 
DEFINITION:  Work Adjustment Training is typically a sixty-day program for adults (age 18 and older) who are handicapped.  The entire program is a system of goal directed services or groups of services directed toward enhancement of the client's job-seeking and job-keeping skills that facilitate movement toward a satisfactory vocational placement.  Some of the possible outcomes are placement, other training, Work Activity Center, Extended Services, etc.


PROGRAM GOAL


The goal for this program is to have a program and staff that can effectively increase the number of clients who move into a satisfactory vocational placement whereby the client has assimilated necessary skills and understanding to do so. 


PROGRAM OBJECTIVES

 
Objectives will vary according to the individual client’s Individualized Written Program Plan.  We attempt to achieve their overall goal by increasing their average productivity, resolve identified problems, increase client's ability to generalize between work tasks, increase personal and social adjustment to the setting and the community, improve other job-seeking and job keeping skills, and determine the need for and extent of additional services or place those ready for placement or refer clients to other programs when their needs can best be served elsewhere.


ADMISSION CRITERIA


The admission criteria is basically the same as that of Evaluation without duplication.  Sponsored Rehabilitation clients will have be authorized (RS-315) by the Rehabilitation Counselor and must be under an approved and signed (RS Counselor) Individualized Written Program Plan.

EXTENDED SERVICES

DEFINITION:  Extended Services is one of the programs offered at Abilities Unlimited of Jonesboro, Inc. for adults, (age 18 or older) who have a disabling condition.  In order to participate in this program the individual must be authorized to receive these services by the Rehabilitation Services field counselor and be under an active Individualized Written Program Plan for Extended Services signed by the client, the case manager and the Rehabilitation counselor.  Usually the client's average productivity is from 40% to 75%.  This program placement usually results from the client's involvement in a specialized training program.  This program operates on a nine month basis, Monday through Friday, 7:00 a.m. to 3:30 p.m.  The clients are involved in various activities related directly and indirectly to the work setting which address specific needs.  This specialized service includes:  counseling and guidance; casework; community orientation and access; skills development; and work adjustment which is intended to increase work habits, ability to work, physical abilities and vocational capabilities.  There are two primary objectives to the program:  competitive employment or further training.  A specific case manager works with this program and helps direct staff involvement with the client.  Individualized Written Program Plans are developed after special needs are identified from an assessment.

PROGRAM GOAL

The program goals for the Extended Services program is to provide special services and training that are planned and directed toward maximizing the individual's vocational, educational, personal, and social functioning in order to achieve the highest level of functioning within the community.  Its ultimate goal is to assist handicapped individuals toward achieving or maintaining self-support activities and to become more self-sufficient.  Long range goals include serving 40 clients annually with 90% completing their overall goal.

PROGRAM OBJECTIVES

Some of the objectives of the Extended Services program are: to assist in personal and social development; to increase production skills; to improve and stabilize specific work behaviors; to provide specific services directed toward assisting the individual to achieve self-support and self-sufficiency; to reduce or control the effects of identifiable barriers; to provide community based programming; and to reduce or prevent dependency.  The objectives are met through a well- developed system of coordinated activities under the work-oriented habilitation and rehabilitation facility which we offer. 

ADMISSION CRITERIA

The admission criteria for Extended Services will be basically the same as that of Evaluation without duplication.  In addition the client will have to be Title XX eligible, under an approved and signed (RS Counselor) Individualized Written Program Plan, and be authorized to participate through an RS-315.
