The Supported Living program is a residential program of Abilities Unlimited of Jonesboro, Inc.  For adults (age 18 or older) who have a developmental disability and may in addition be emotionally and/or physically disabled and are in need of specialized services.  The program operates 365 days a year on a 24 hour basis.  The individuals are involved in various activities which address specific needs providing: habilitation training; diagnosis and evaluation; transportation, personal and social adjustment sessions, community resource awareness and use , and counseling and guidance.  A Case Manager and Coordinator, case manager and coordinator assistant, and team leader program implementors are the facility staff that work directly with the individual along with the Director of Consumer Services and other professional support staff.  Individualized program plans are used with each individual so as to be able to work on very specific needs.  This program provides transportation and three daily meals, and is designed to meet the needs of those who need supports to learn to live more independently in the community.

PROGRAM GOAL


The program goal of Supported Living is to provide special modified living services and related training supports that are specifically planned and directed toward supporting each individual to develop the necessary daily living skills and self-care skills in order to achieve the highest possible level of independent
living in the community.  In short, these modified living services are intended to lend supports to the individual to become or remain self-supporting and more self-sufficient through community based programming in an environment that can offer protection for individuals who are at high risk.



OBJECTIVES

Some of the objectives of the Supported Living program are:  to assist in the development of daily living skills; to assist with self-care skills development; to assist in personal and social development; to provide specific services directed toward assisting the individual to achieve independent living skills and self-sufficiency; to reduce or control effects of barriers; to provide community based care; to support consumers in learning and using community resources; to support the individuals to become self-supportive to the greatest extent possible.   These objectives are met through a wide variety of activities in and around the Personal Adjustment Center that deal with those specific areas necessary for the development of independent living skills. 



ADMISSIONS CRITERIA

Special admission criteria applies to this program including having to be age 18 or older, a documented disability, self-care skills, to be ambulatory or self-sufficient in a wheelchair or other prosthetic devices or aids, make rent payments and more.  If you are interested in these services, please obtain this admissions information to see if it fits with your needs.
