INTEGRATED SUPPORTS-COMMUNITY BASED
This is a program designed to provide support to individuals with severe disabilities who require extensive on-going support in more than one major life activity in order to participate in integrated community settings and to enjoy a quality of life that is available to citizens with no disability. Support may be provided for life activities such as mobility, communication, self-care and learning, as necessary for independent living, employment, and self-sufficiency. It is designed to give our community-based consumers support to maintain their independent living situations. Implementors provide one-on-one assistance to consumers in developing skills to increase their quality of life. They receive assistance in transportation, socialization, recreation and accessing community resources. Counseling and case management is provided by the designated coordination for this service.The program relates directly to the portion of our mission that involves improving the living conditions of people with severe disabilities. However, it often relates to work because the implementors also help their consumers fill out job applications or instruct them on job retention skills. It also relates to many of our core values. These consumers give the final approval for implementor assignments. They meet and discuss their goals with a prospective implementor before that implementor is chosen to work with them. They also pick their own goals. This program provides continuous support to our consumers, but the support is always as minimal as possible, and focuses on the consumers individual needs.
The goal for this program is to provide direct support and services to persons who have moved into their own residences to help them maintain in those environments to the best of their abilities. The services of financial, counseling, transportation, and service acquisition are specifically designed to their individual needs to help achieve that goal. Through assessment, case plan development, and supportive services the individual is assisted to become or remain self-supporting and more self-sufficient through community based programming. Some of the objectives are: to assist the individual with assimilating independent living skills through planned training and intervention; to assist the person living in the community in an integrated fashion; to assist the person to recreate in an integrated environment; to reduce or control the effects of barriers; to reduce or control dependency; and to assist the person to meet the overall demands of wholistic living in an integrated environment.
